Try the labor board, the better business bureau.

First you really have to know whether they are reporting the money as income. If they are and paying all applicable taxes, then no crime has been commited. If they are not, then it is income tax fraud.
Also you need to know whether or not insurance claims are being filed for these services, then that would be medicare/medicaid/insurance fraud.
Most doctors have office managers to account to, and it would be hard to circumvent someone in that position. If you believe there is dishonest business practices afoot, then it is most assuredly not solely the physician. There are other hands involved in this.
The BBB does not really investigate, it merely notes when an incident/complaint has occured. The labor board will only help in the eventuality this becomes a labor/management case.
Consult the sheriff's department in your area, and see what they advise. See if there is a whistleblower's number you could call about income fraud.
Then look for a new job.

I no longer work there, a lot of the problem is patients paying the doctor and he is STILL filing insurance so therefore the patient has a credit that they NEVER get.....things are never written on charts when services are performed, and usually a N/C (no charge) is written in the price column, but money is being passed The office manager has NO guts to stand up to this, there is a fee list but he doesn't abide by it, he just charges whatever he wants, and there is different charges depending on the race of the patient
